Faculty Intake Form

Please complete this form if you are a faculty member and require access to the OHSU Campus. This form should be completed at least 1 month prior to OHSU access. All faculty members are required to have an up to date OHSU ID badge and require module completion on an annual basis. If you require additional access including Epic, you may be required to attend additional training.
