1. Only items documented through this service request form will be accepted. Separate lists, verbal communications, and any other form outside of this form submission will not be accepted.
2. Technical assistance will only be provided during normal business hours. (Monday - Friday 7:00am - 3:30pm PST)
3. The technical assistance representative may request the apparatus be inspected in person at any point during the exchange of information in which case a work order would need to be created.
4. Golden State Emergency Vehicle representatives are not responsible for damage(s) to apparatus and/or property as a result of technical instruction.
5. Additional assistance may be requested from the apparatus manufacturer which can lead to longer than normal resolution times.
