Maryland Stars of Life Awards Nominations

Each year, the Maryland Institute for Emergency Medical Services Systems (MIEMSS) celebrates EMS Week by honoring men and women across Maryland who have contributed to the EMS system. The categories for the awards relate to specific contributions and events occurring in each calendar year.

Nomination Information:

To nominate an individual, group/team, or program, you will need to provide the following items on this form:


  1. Select the category (drop-down menu).
  2. Enter the information for the nominee to include name; contact information; address; and a nomination description (reason for nominating).
  3. If the category you picked has the option for multiple awards, select the number of additional contributors. Include their name . relevant position information, and describe the extraordinary nature of their contribution.
  4. Use the uploader to share any additional information or supporting documentation.
  5. Provide the nominator's name and contact information.
