Complete this Smartsheet and the available spreadsheet to report Supplemental and Contracted Staff for Physical, Social, and Emotional Support (PSES) Staff compliance for the 2024-25 school year.


First, complete the first four questions.


For question #5, if "Yes", complete the spreadsheet (click on available link). Information on how to complete the spreadsheet and when to submit is provide on the "Narrative" tab of the spreadsheet. Upload the spreadsheet in the #6 - File Upload box.
