To ensure your question(s) get answered, a CDA Office Hours Form is now required! All inquiries will be added to the agenda and addressed in the order they are submitted.


You do not have to submit a form to attend.


Purpose of this form:


  • Organize real time data abstraction questions
  • Create a CDA facing FAQ
  • Ensure all CDA's needs are being met
  • Promote problem solving and critical thinking skills
  • Promote collaboration and peer to peer mentorship
