Extension Request

The faculty PI should complete this form if an extension is needed beyond the two years mentioned in the award letter.


Please note that making a request does not guarantee that an extension will be granted.


You will be notified of a decision within two weeks.

In detail, please provide the reasons for your extension request.

Please provide a revised plan timetable for your project.

Please enter date by which you anticipate you will be able to submit the final report. A reminder will be sent two weeks in advance of this date.

Please provide a short update on what has been accomplished.
