Please complete the form to be entered into the quarterly drawing for JCH swag!

The program will start July 1, 2022, with quarterly drawings being held the first week of October, January, April and July. Winners will be sent a link, gift code and instructions on how to select JCH swag that is exclusive to this program. If selected, we will verify participation in activities before an award is given.

Eligible Activities;

• Share a Jersey Community Hospital Facebook post to your personal page/timeline.
• Review Jersey Community Hospital on Indeed.com.
• Review your JCH Provider on Google after an appointment.
• Volunteer at a JCH hosted event or represent JCH at a community event. (For example; Volunteer at JCH Recycle Day, Walk in a parade with JCH, Volunteer at JCH Foundation events, etc.)
