Student Services Request

Please ensure your student service request includes all required fields and the necessary forms listed below related to your request:

• Enrollment Verification, & Transcripts requests must include the 'Transcript Request Form'
• VA Enrollment must include your current Certificate of Eligibility letter from the VA and a copy of your DD-214
• Student Educational Accommodations - must include the 'Accommodations & Verification Disability Form'

An additional $25 fee will be collected, if not yet paid, for services such as transcript requests and duplicate certificates. Please note that credit/debit card payments are subject to a 2.75% processing fee, with a minimum charge of $3.00.
