The student complaint policy is designed to help students address issues at the lowest level in the most direct and least taxing way possible.
Complaints Involving Faculty

Step 1: Discuss the situation directly with the instructor.
Step 2: Discuss the situation with the department chair.
Step 3: Complete this form. It will be routed to the associate dean for review/discussion.
Step 4: Discuss the situation with the school dean.
Note: You will be asked to resubmit your complaint if you do not complete steps one and two before submitting this form.
