1 - Please submit a Truck Setup form for the temporary truck that you're bringing online to use this Mobile Device with

• Please note that Mobile Devices are reserved for use with temporary trucks in the event of a breakdown of a truck with existing telematics only.

2 - Please fill out and submit this form, before making contact with Coillte Accounts Receivable (Coillte AR).

3 - Once submitted, you will need to contact Coillte AR to provide payment of a deposit for rental of a mobile device from your specified weighbridge. This deposit (€150 - as referenced in the CoPTR, page 16) is for rental of a device for a maximum period of 7 days.

4 - Coillte AR will confirm (to Trimble Forestry) receipt of your deposit and we will be able to proceed with setting up your temporary truck, keyfob and mobile device.

• The keyfob from your existing truck (which is going off the road for repairs/etc) will be moved onto your temp truck for the duration of the rental of this mobile device.
• The truck going off the road will be banned until such time as it is ready to return to work.

#NB - You will incur penalties or mobile device usage restrictions if you fail to adhere to the usage and return policies outlined in the CoPTR (page 16).

Note:

Please be aware that the mobile device must be returned to the weighbridge from which you collect it in the same condition in which you received it - with all peripheral attachments, cables, etc safely contained within the Pelican case that the device comes in.

• If any damage occurs while it is in your care, please report this to Trimble Forestry and the weighbridge from which you are renting the unit.
