Computer (Laptop) Request Form for Classified and Certificated Staff
***When requesting a laptop keep in mind this will replace your desktop. Your desktop will be removed when your new device is delivered. There are no exceptions, laptops are replacing desktops going forward.
The District allows 1 laptop per filled FTE.
When requesting a laptop, please keep in mind the following:
- Requesting a Replacement for a 5 year or Older Computer: Refresh eligibility is based on age of device - it must be 5 years or older from date of purchase.
- Requesting a Laptop for the First Time: This would be for a new employee or position. Vacant positions are typically ineligible for requests.
- This Form Can be Filled Out on Another's Behalf: Please note that the employee who is receiving the laptop must schedule delivery with our vendor, and be available at the time of delivery to complete the setup process.
**DO NOT FILL OUT THE FORM MORE THAN ONCE PER REQUEST.
- Requests will be vetted and approved based on the guidelines above.
