Complete this form to add, move, rename, or inactivate sites in the Colorado Nutrition Portal. Requests must be submitted by the sponsoring organization or School Food Authority. CDE School Nutrition may approve site requests for sponsors participating in the National School Lunch Program, School Breakfast Program, Special Milk Program, Summer Food Service Program, and Seamless Summer Option.

If you need to request changes for a Child and Adult Care Food Program site, please contact the Colorado Department of Public Health and Environment at cacfp@state.co.us.
