Please complete this announcement form at least two weeks before your presentation. If you have any questions about your presentation and/or this form, please contact cs-office@virginia.edu.

Include the building and room number, plus Zoom link if applicable.

Include their organization/institution if not at UVA

Upload the CV in the file upload section at the end of this form.

The title entered here is what will be used for your announcement and paperwork sent to the registrar. Please make sure your title is correct.

The Committee Appointment form should be submitted for the Qualifying Exam and Dissertation Proposal, and resubmitted any time it is revised.


The Final Examination Committee form should be submitted prior to a student’s final thesis or dissertation defense.


The GPC will request the Program Director's signature on your behalf.
