Phase III Grade Appeal Submission Form

We take the process of assigning grades to medical students very seriously, and strive to make fair and balanced assessments of each student’s performance using all available information. If you have questions about your assigned grade, please speak with your Block Director. After this conversation, if you believe your assigned grade is not a fair representation of your work compared to your peers, you have the right to appeal this grade to the Block Director within 28 days of receiving your grade. We recommend you discuss a possible appeal with the Block Director and/or an Assistant Dean in the Office of Student Life informally prior to submitting a written appeal. Please review the full CUSOM Grade Appeals Policy in the CU SOM Student Handbook for additional guidance: https://medschool.cuanschutz.edu/docs/librariesprovider31/education-docs/ome/2021-2022-policies-and-procedures_-hybrid-and-legacy-curriculum-final.pdf
