Transfer from Limited to Full Licensure

To transfer from Limited to Full Licensure, please upload the following:

SLP Limited to SLP Full: Completed AS3 Form and Praxis Scores or ASHA Certification and $150 Fee (AS3 form found in the SLP Limited Application)

SLP-A Limited to SLP-A Full: Completed SA7 Form and $100 Fee (SA7 form found in the SLPA Limited Application)

AUD Limited to Full: Proof of Praxis Scores and $150 Fee

HAD Limited to Full: Once the Board's Practical Examination and the International Hearing Society's (IHS) Written Examination have been passed, submission of a $100 Fee will be required. Completed HA5 Form is also required.
