CommunityID Enrollment Request

At Rutgers University, this form should be completed by service providers (SPs) to enroll and request integration of service(s) with CommunityID, allowing said services to be available to users via a CommunityID account at Rutgers, after approval and successful integration. CommunityID is an account for a broader, extended community of users at Rutgers University . Users that are part of an extended community are those who are not currently affiliated at Rutgers as faculty, students, staff and/or sponsored guests . With a CommunityID account, users can register/manage their account in order to access specific extended community services, that are distinct from regular services for affiliated users available with a NetID account at Rutgers. For more information about CommunityID, go to: https://rcm.rutgers.edu/aboutus.html NOTE: CommunityID at Rutgers University implements identity management (IdM) and single sign on (SSO) via Shibboleth and SAML (or Security Assertion Markup Language) which allows for the exchange of identity info between identity providers (IdPs) and service providers (SPs). With regard to the form fields below, CommunityID is considered the Rutgers IdP and your service/application/system is considered an SP. Form fields below with an asterisk (*) at the end for the form field name are required in order to submit this enrollment request form.

For example: If accounts are provisioned via a data feed/batch file, then describe how frequently the data feed is received and method of data transfer (e.g., secure transfer, SFTP upload, etc.). The Entity ID is a globally-unique identifier (usually in the form or a URL) which is used by the IdP to differentiate between different service providers. Two or more service providers cannot have the same Entity ID. If a conflicting Entity ID is submitted, it will be rejected until the service provider revises it to a unique value.

The endpoint URL for your service in a TEST/DEVELOPMENT environment where CommunityID as the Rutgers IdP will send the SAML response.

As the primary initiator requesting integration with CommunityID at Rutgers, I acknowledge that it is my responsibility to ensure that the Service Provider (SP) understands and adheres to any and all applicable local, state, federal, and/or institutional policies governing the privacy and protection of any data released via CommunityID. Specifically, the SP acknowledges and understands that it shall undertake any necessary process or procedure for the continuous safeguarding of any information received via CommunityID, using best practices including, but not limited to, encryption at rest, physical/logical security compartmentalization, appropriate network/firewall restrictions, etc. The SP also acknowledges and understands that it shall not share, disclose, rent, sell, disseminate or use for any unauthorized purpose any data received from CommunityID with any other parties without the express written permission of an authorized agent on behalf of Rutgers University.

Please note that submission of this enrollment request form does not automatically guarantee integration with CommunityID at Rutgers University. All submissions will be reviewed and may be subject to additional approval. We may also contact you to request further information or documentation regarding the SP's data protection and privacy policies. You will be contacted when a final decision has been reached.
