The hotel contract will be based on the number of hotel rooms provided below.

Please provide the date, time, and number of guests for each requested reservation. Also, please include type (e.g. Steakhouse, Italian, etc.) and logistical requests.

This will be used to title the meeting in Zoom.

Include the meeting's start time, end time and time zone. If held over multiple days, please include the start and end time for each day. (Example: 5/8/21 - 10:00 a.m. to 11:00 a.m. Central Time)

Quick Poll Request Form

Please complete each of the questions below regarding your Quick Poll/Survey, and allow up to three (3) business days for your request to be completed.


Once your template has been created, you will receive an email that includes a link to test and approve the Quick Poll/Survey. Note: Quick Polls/Surveys will not be launched without receiving prior approval from the requester. The start and end date may be adjusted based on requests currently in the queue.


At the close of your Quick Poll/Survey, you will receive a report detailing the results.

Video Project Request Form

Whether promoting your Board of Accountancy’s exam application process or educating licensees on enforcement, NASBA’s Communication Department can produce custom multimedia projects to suit your board’s/department's needs. Projects range from the production of public service announcements (PSAs) and promotional/whiteboard videos, to voice-over and editing services. To get the process started, please follow the steps below.


  1. Complete the following Multimedia Request Form, submitting one form per video request. Once complete, click on the Submit button below.
  2. Once received, the content will be reviewed, a pre-production meeting will be set up to discuss details (i.e. talent, location, due date, etc.) and a production timeline will be established.
  3. Once the shoot and edit process is complete, a draft will be sent to you for review and final approval. Clients will be given (2) two opportunities to submit revisions.
  4. If edits are needed, the client is asked to submit an email detailing all needed revisions. If the project does not require edits, the client is asked to submit final approval via email. Once approved, the project will be sent to the client via email, with a link added to NASBA’s Vimeo page.

Every email needs a subject line that summarizes what is in the body of the email.

Main headline will appear in the body of the email. It may be different than the subject line.

Email preheader text is a small line of text that appears after the subject line in an email inbox. Email preheaders give a short summary of the contents of an email, and may appear differently on mobile and web email clients.

The 'from' line of an email shows the recipient who sent it. For example, it could be the company name, a Board of Accountancy, a person's name.

The 'from address' is the email address that is sending the email. It will also be the email address where replies are sent.

Please note: To give reasonable time to receive, process and begin work on your request, our staff has a 72-hour turn around time to send the first 'test' email for review and approval.

Note: When selecting one or more of the lists from the NASBA Database, new lists will be pulled prior to distributing each Survey/Quick Poll to ensure data accuracy. When using a Custom List, the requester MUST specify and provide the most current distribution list (Excel format is preferred). We are unable to fulfill requests stating "pull the last list used.”
