Dear Volunteers, at Levy, we are committed to creating memorable experiences for each of our guests. Levy volunteers have a big part in ensuring that we meet that expectation while serving safely and responsibly. To ensure you have all the tools, volunteers are required to register and to complete our online training program and sign receipt of Levy policies annually.

The following files are required. COMPLETE THESE FIRST and then come back to this page to upload all at the same time.

1. MAST Permit (temporary is acceptable if current)
2. Food Handlers
3. Volunteer Workbook & Annual Policies signed
4. Levy Training Certificate
