FS Request for Shutdown Notification

When a utility shutdown is needed for a renovation or repair, please submit the following details. A copy of your submission will be sent to you and Erin Chapman / erin.chapman@ucr.edu.


NOTE: Please review the following guidelines before submitting a request:

  • PD&C Requests – this form does not replace the USR form process, but can be completed in conjunction for administrative support
  • Minimum 2-weeks’ notice for all planned maintenance repairs
  • Minimum 30-days’ notice for large impact shutdowns affecting research and students (e.g., all-day shutdown for electrical utilities in a research location)
  • Emergency, unplanned repairs occurring within 72 hours that have no flexibility are considered Urgent notice (e.g., leaking main water line, elevator outage)

Specify the work to be performed (repair failing equipment, reroute, install, replace, etc.)

Specify the impact(s) to building occupants using simple, precise language. What should they be aware of?

Check box if shutdown is required to occur in the next 72 hours with no flexibility.

NOTE: A minimum of 2-weeks' notice is needed for maintenance repairs. A minimum of 30-days notice' is needed for large-impact shutdowns affecting research and students (e.g. all-day shutdown for electrical utilities in a research location).

Check box if shutdown will impact Housing or Dining facilities on campus. (Minimum 48-hour notification is required for all tenant impacted shutdowns.)
