For good and valuable consideration, I give permission and grant the Office of the Governor (“GOV”) the right to use my story, quotes, and photographs of me; and


  • The right to share my name as an attendee, participant, and/or presenter.


By signing this form:


  • I release GOV from all claims which I may have in connection with or arising out of GOV’s use of these.


  • I represent that I have the right to display and share all content made available by me during the meetings and presentations associated with GOV.


  • I understand that the use of my photograph, image and/or voice may potentially be used in the following ways:


  • GOV internet accessible via the World Wide Web to anyone.
  • Uploaded to Facebook, Twitter, Instagram or other social media platforms that pertain to GOV activities.
  • Other uses in GOV’s discretion, including trainings, GOV materials, showcases, email newsletters, and other GOV-related activities that may be publicly accessible via the World-Wide Web; and
  • GOV Internet


  • I agree to be bound by any related contractual obligations that may exist between GOV, my employer, and myself with respect to the GOV and its activities.


  • I relinquish any rights to the quotes and photographs provided and understand that they may be copied, used, and published by GOV without further notice or permission, for any lawful purpose, including, but not limited to, training, education, and informational purposes in any media.


I confirm that I have authority to give consent to be photographed and recorded and be held accountable for the presentation materials as stated above. I understand that any information I provide could be considered a public record and may be subject to disclosure under the Public Records Act, RCW 42.56.
