This form is for LEA leaders that missed a REQUIRED monthly LEA CoP. One representative must be in attendance for an LEA to be marked as present.

The representative will:

• Watch the recording of the CoP
• Review the PowerPoint Slide Deck
• Complete this form

All resources can be found on the Interactive Calendar on the Strong Foundations Planning Website.

1. Double-click on an event on the calendar. CoPs are offered Thursdays and Fridays monthly. You only choose one to view.
2. Scroll down in "View Detail" window.
3. You will see the slides and recording links.

This must be completed within 3 weeks of the missed CoP.
