Transfer Requisition (T-REQ)

• Make sure all items are ready to go in the listed location prior to submitting your request


• Make sure all boxes are secured with packing tape


• Place a copy of the transfer requisition on every box/item listed. This includes shred, discard books, curriculum, technology items, furniture, office supplies, etc.


• Make sure all areas of the t-req are completed with the requested information including appropriate signatures or it could be rejected.


• Do not reuse old T-Req forms or cut and paste signatures. Each new request requires new approval signatures.


• HP & Dell Computers should be packed separately and listed separately on the t-req form.


• Disposal of any printer/copier/fax machine requires an “Approval to Dispose” email from Melanie Webster in purchasing. Please include a copy of the email with your request.


• Currently, we are not accepting any type of “computer carts” per DoIT.


• For shredding/discard of any records, non-records, books and textbooks, please include the approved CORD form and weeded book/curriculum disposal log along with the approved T-Req. https://www.bcps.org/system/records_management_program
