This form is to be completed only after the Employee Portal on MLink is closed for updates. Employees can make any correction, addition, cancellation or adjustment in the portal up to the Wednesday following any pay date. Report leave in full, or half day increments. SUBMIT ONE FORM FOR DELETIONS, AND ONE FOR ADDITIONS.
