Submit an Inquiry

If you are unsure whether the Secondary Data Use Review Committee needs to review a case, we welcome you to submit an inquiry via this form for the SDU Staff to assist in the determination.


If all 12 guiding principles are met (there are no violations of any of the principles), there is no need for the SDU Review Committee to be involved. Please proceed as usual.


If one or more of the 12 guiding principles are clearly violated, please bypass this inquiry form and submit your request for review via the full Review Request Form.
