A project is considered sustainable if it can be completed or maintained without requiring ongoing financial contributions from UHA.


A project is NOT considered sustainable if it depends on future financial support from UHA for its completion or continuation.

Projects are given priority if they:


  • Connect with more than priority population
  • Are equity driven
  • Incorporate cultural and linguistic responsiveness
  • Support one or more priority areas and specific objectives outlined in the UHA Community Health Improvement Plan (CHIP)
  • Have not previously received grant funding from UHA


CHIP Grant funds are not permitted to be used for the following purposes:


  • Wages
  • Advertising
  • Covered services for an OHP member
  • Administrative activities to support the delivery of covered services
  • Coordinated Care Organization (CCO) contractual requirements
  • Provider workforce or certification training
  • Building new buildings and other capital investment activities
