Use this form if you:

Are a physician
Need to obtain a faculty title as part of the credentialing process with Banner - University Medical Center Phoenix (B-UMCP)
Will not be involved in teaching medical students, residents or fellows

If you need to apply for a teaching title, please visit: https://faculty.phoenixmed.arizona.edu.

Once your form is submitted, the appropriate faculty coordinator will review the request. The department chair or division chief must approve all faculty physician title requests.

You will receive a status update at the provided email once we hear back from the chair/chief. This process may take one to two weeks.
