CSM Club Information Form

Please complete this form to register your club for the current semester. When submitted, requests for approval will be sent to the following individuals (in the following order):


  1. Club President
  2. Club Treasurer
  3. Club Advisor
  4. Student Life and Leadership Manager


When your club is approved by the Center for Student Life, you will receive a separate confirmation email.

E.g., weekly, every other week, 1st and 3rd, 2nd and 4th, etc. Please note that clubs must meet at least twice per month.

Please list up to three (3) on-campus locations that you would like to meet. The Center for Student Life will check on availability in the order in which they are listed. No reservation is considered official until separate written confirmation is provided by the Center for Student Life.
