  1. Amount - Dollar amount of request
  2. Justification - Attach your justification below and include ALL potential sources of funding, including start-up funds.
  3. Duration of Project - Timeline of project
  4. Space Needs - Describe the space needs for the project or shared instrumentation
  5. Prior Research Office Funding - Indicate if you have received funding from the Research Office before. If so, how much and when?


Note:

  • Shared Instrumentation - Any shared instrumentation request that is greater than $100,000 requires more than 3 users. Please attach a quote below if the requested amount is greater than $100,000.
  • Pilot Projects - Pilots must not exceed $10,000.
  • Personnel costs are excluded

Lab Access Guidance

Please note:

  • The Translational Research Office is only able to help modify access levels on a badge. To acquire a badge, contact your Director of Operations for assistance.
  • Training is required to gain access . Lab Access requests will not be considered until the person requiring access has completed the four virtual prerequisites and enrolled in Lab Safety Orientation.
  • If this request is related to a DCC (new or change) please first complete the DCC form. Do not request lab access prior to receiving an Employee ID.
  • To terminate badge access please complete a work order.





Lab Safety Orientation Enrollment Instructions

LAB SAFETY ORIENTATION IS NOW IN EDGE LEARNING. PLEASE USE THE LINKS BELOW TO ENROLL.


Please enroll in the courses listed below in Edge Learning prior to enrolling for Lab Safety Orientation. It’s best practice to complete these four virtual prerequisites prior to attending in-person Lab Safety Orientation.


1. Bloodborne Pathogens and Universal Precautions (OSHA Compliant)

2. Basic Biosafety Protection Course

Must complete Bloodborne Pathogens and Universal Precautions prior to taking this training.

Offboarding

The Translational Research Office can assist with offboarding for local lab-related tasks. To terminate badge access please complete a work order. More resources are available at the below links.


Make sure to complete the Departing UAHS Investigator Form for assistance with grants, IRB/IACUC protocols, and transferring assets.

Visit the iLab core scheduling page for more information regarding reservations, registration, account numbers, and interlocks. Please include a detailed description of your request (core, instrument names, etc.)
