Any person or organisation using ISA services or affected by its operations has the right to lodge a complaint. This can also be done anonymously by indicating this preference below.


This Form is for lodging a COMPLAINT or STAFF GRIEVANCE.


A separate Feedback Form is available for any feedback that is not related to a complaint.
