Smartsheet License Request

Please review Penn's Data Classification and Smartsheet's privacy statement when deciding what data should/should not be stored in a Smartsheet. Please exercise thoughtful stewardship of Penn's data.

This form can be used for the following purposes:

1. Request a new Collaborators License (free)

2. Request a new Full License ($)

3. Upgrade from an existing Collaborators (free) License to a Full License ($) - SEE NOTE BELOW

4. Downgrade from an existing Full License to a Collaborators (free) License

5. Remove/disable an existing Smartsheet license


NOTE: When transitioning from a trial or personal contact account to a licensed account under this plan, please use one of the NEW request options listed above (#1 or #2). This transition CANNOT be processed as an upgrade (option #3).

What will your role be on the work/project?*

Full licenses of Smartsheet are charged back at a rate of $28.33, to a 26-digit budget code or a Health System Lawson code, on a monthly basis.
