Remote Work Participation Agreement
This form should be submitted by the Employee. Remote work means an arrangement by which an employee works from a remote location other than the primary worksite, either for part of their schedule, or entirely. Participation is voluntary subject to the university’s approval. Participation is not an employee right or benefit, and may need to be discontinued for business reasons. Regular office means the employee's usual and customary work address. It remains the primary work location even if participating in a remote work arrangement. Please note: Please use this form to submit a new remote work request for your current position. This form does not carry over to another role. A new form will be required if filling out for another role. If you wish to amend an existing agreement, you will need to submit this form again. Student Employees are not required to complete this form.
