Currently, this agreement is open to any U.S.-based academic, nonprofit, or health care organization. Please complete this form so that our team can work with your signing official to execute this agreement. The agreement will be with the American Heart Association, Inc., a New York not-for-profit corporation, having its principal offices at 7272 Greenville Avenue, Dallas, Texas 75231-4596 ("AHA").
