SWPPP Pre-Permit Form

The completion of this form is required to determine if a full Storm Water Pollution Prevention Plan is required for each project site within the City of Southlake's border based on TCEQ requirements. Be sure to read and submit each item carefully. Once a form is completed it will be submitted automatically for a review, which has a maximum turnaround time of 10 business days. Once it has been reviewed, a Pre-App letter will be emailed back using the contact information you provide. If you have any questions, please email Madisson Dunn at MEDunn@ci.southlake.tx.us.


Section 1 - Basic Information

Name of the project.

If seeking coverage of multiple addresses under one SWPPP, list all parcels/addresses below.

City of Southlake Permits - Requested*
Is this a New Permit or a Revision?*

Provide name of company or business responsible for the project.

Provide the day-to-day construction site owner/operator.

Provide a number for the inspector to contact the on-site operator anytime.

Provide a number for the inspector to contact the on-site operator during business hours.

Provide an email address to receive the acceptance letter and further information when completed.


Section 2 - Status

READ CAREFULLY! Incorrect information may delay this review or may cause you to be liable for fines from the City and/or the Texas Commission on Environmental Quality. Your activities are regulated under the Texas Commission on Environmental Quality (TCEQ) General Permit TXR150000 based on the size of the project. Some projects are required to take further action. You are responsible for determining whether further action is required to comply with state law. Below is basic information about the Construction General Permit that will assist you in determining your status. Common Plan of Development*

Is this a common plan of development as defined by TCEQ? Will there be concurrent work now or in the near future within 1/4 mile of the site listed here by the same entity?

How many acres are being disturbed during the life of the project? (ex. 3.4 acres)


REMEMBER! If this is a Common Plan of Development, all disturbed area within 1/4 mile must be added together for this value.

A Site Plan and Grading Plan showing the area disturbed must be uploaded as part of the submittal.
