IT Project Intake Form:

Any initiative or request which would require defining requirements, scope, budget, and resources. Projects are assigned a Project or Program Manager to monitor, track and report progress, risks, issues and schedule. Most projects require a team of resources to develop, test and release the new or change to existing technology and are completed over a period of weeks to months to years.

Task Request

Requests requiring IT support but are handled quickly and do not require the same level of support as a project. This form will collect specific details based on the work needed. By separating these requests from the larger project requests, we are able to streamline the workflow, assignment, management and reporting of this work more efficiently.
