Students with Service Animals In Training Acknowledgement Form

On a semester-by-semester basis, Clemson University allows Service Animals in Training onto University property upon completion of this regulated acknowledgment form. Any student or student group wishing to bring a Service Animal in Training onto University property must first submit this intake form, supplying all required documentation and acknowledgments. Please note that this acknowledgment process is complete upon form submission and is valid for one semester. It should be submitted prior to the start of classes and updated in the event of any changes.

Please note the following list of requirements for form completion. Without a "yes" acknowledgment AND submission of specified documentation, you will not be able to complete this process:

  3. Proof of $1M liability insurance documentation (upload)

Service Animals in Training must have a general $1M liability insurance policy provided by the agency or program sponsoring the training and be engaged directly in learning service tasks.
