Foster / Foster-to-Adopt Application

German Shepherd Dogs can be challenging, high energy and protective of their home and family. While these are often their most endearing qualities, they can also make them hard to live with! Please read the following statements and check the box next to them to acknowledge that you accept them.

PLEASE NOTE: Completing, signing, and submitting this application constitutes your permission for us to check references and confirm any and all information provided in this application is accurate. Both parties agree that this application can be executed by the applicant and sent to True North GSD Rescue via this online form. If the applicant would like to submit this document via this online form, applicant and True North GSD Rescue both agree that the applicant’s signature is valid and binding upon the parties. If you would like to submit an application by US Postal Service, please email rescue@truenorthgsdrescue.org requesting a printable application. As part of the adoption process and before placing any dog, you will be asked to sign a contract in which you agree to certain conditions including keeping the dog as an indoor house pet, maintaining regular veterinary care, allowing a rescue representative to visit your home before and/or after placement of the dog, and finally, surrendering the dog back to True North GSD Rescue (not animal control nor a shelter) if you violate the contract or are unable to keep the dog for any reason.

I certify that I am at least 18 years of age and that I will ultimately and solely be responsible for the care and well-being of any dog I foster or foster-to-adopt from True North GSD Rescue. Any misrepresentation of the true facts in this application or in the adoption agreement will invalidate the adoption agreement and will give True North GSD Rescue the legal right to reclaim the dog immediately without monetary or material compensation. If I foster a dog from True North GSD Rescue, the dog will reside in my home as a pet. I will provide him/her with adequate water, a high quality dog food, shelter, training, affection, and medical care.

I understand that True North GSD Rescue is a referral service and is not responsible for the accuracy of information received about the habits, temperament, or physical condition of the dogs available for adoption. I understand it is my responsibility to evaluate a dog for myself before agreeing to foster him/her.

By affixing your signature below, you authorize True North GSD Rescue to conduct verification of the information provided in this application, including, but not limited to, criminal history, employment verification, reference checks, and other pertinent inquiries. You acknowledge that the gathered information will be utilized exclusively for assessing your eligibility for fostering and/or adoption. I am in full agreement of the terms of the fostering. True North GSD Rescue shall not be liable for, and is hereby relieved from, all liability for any damage, expenses, causes of action, fines, suits, demands, judgments, or claims of any nature whatsoever arising from or by any reason of any damage to property or injury to any persons caused in whole or in part during the visitation of the dog up for adoption and a dog placed in your home.

I hereby accept and assume such liability and agree to protect, indemnify and hold True North GSD Rescue and all representatives, agents and volunteers harmless from and against all of the aforesaid.
