Project Close Out Document Collection Portal

This portal is intended to collect any documents related to the closing out of all childcare projects that were funding with LISC OEC Facilities Construction and Renovation Grant Program dollars. Any unrelated documents uploaded to this portal will be discarded. If you have any questions related to your submission, please contact us at CTOECFUND@LISC.ORG.

Your program's PA# can be found at the very beginning of your Grant Agreement with LISC.

Project Update Summary

You are required to update LISC as your project progresses and completes. Please indicate below if your project is complete and provide updates on the status of your project.

Have you collected all of your paid invoices and receipts for the work you completed and equipment you purchased?*
Did you take before and after photos of your project?*

This includes if your project is complete, if you still have inspections to be completed, if you still have items being shipped, and any other circumstance you may be in.

Project Close Out Documents

THE FOLLOWING DOCUMENTS/ITEMS ARE REQUIRED TO BE SUBMITTED TO LISC ON OR BEFORE 15 CALENDAR DAYS AFTER THE GRANT'S EXPIRATION.


IF YOU DID NOT REQUEST AN EXTENSION TO YOUR PROJECT, YOUR GRANT WILL EXPIRE ON DECEMBER 31ST, 2024. THIS MEANS YOU WILL NEED TO SUBMIT THE ITEMS BELOW ON OR BEFORE JANUARY 15TH.


IF YOU DID REQUEST AN EXTENSION TO YOUR PROJECT, YOUR GRANT WAS AMENDED TO REFLECT A NEW EXPIRATION DATE OF APRIL 30TH, 2025. THIS MEANS YOU WILL NEED TO SUBMIT THE ITEMS BELOW ON OR BEFORE MAY 15TH.


IF YOU RECIEVED A SECOND EXTENSION WITH AN END DAT OF MAY 31ST, 2025 BE PREPARED TO UPLOAD THESE DOCUMENTS ON OR BEFORE JUNE 15TH, 2025.


  • Proofs of Payments to Contractors and Vendors (Paid Invoices/Receipts of Final Payments to Contractors and Vendors)
  • Photos/Images of Project Completion
  • All Permits Acquired from your Local Zoning Agency
  • Final Inspection Report for Each Permit Pulled
  • All Approval Documents from your town's Fire Marshall
  • All Documents from your Architect/Construction Manager Regarding Project Completion
  • A Copy of your Change Order Submitted to the OEC Reflecting All Changes Addressed with your Project
  • FOR NEW CONSTRUCTION PROJECTS ONLY: A Certificate of Occupancy
