User Access Request Form
All below information should be based on the employee requiring access.
The first name of the person who needs access.
The last name of the person who needs access.
Please provide the name of the portfolio manager or direct supervisor for access approvals. Certain product requests require approval.
This is the employee specific email. Requests with the Info email address will not be processed.
For those requiring access to multiple properties, provide the primary property's direct line phone number. Cells and lead tracking phone numbers should not be used.
For PayScan Training requests to gain access, please submit tickets via a separate HelpDesk ticket.
Do not include platform or property access information outside of the temporary access.
This is the date the access should be removed. This would not impact the users primary access. If no date is entered, a new ticket to remove access would be required.
Check the box for each product access needed. Only check the box for platforms utilized at the property.
