Please fill out this form to directly request funds to purchase any capital equipment from CFAC and LCCC. Completion of this form also must occur ahead of soliciting support from UNC SOM departments outside of your home department. If you have identified other departments or centers as possible contributors to this request due to the nature of the customer base, please elaborate where requested to receive approval and guidance on soliciting those funds.
