SCRIP One Time Request Form

Use this form to make a one time request for one or multiple physical gift cards. Please complete a separate request form for each brand of gift card (I.e. Amazon, Giant, etc.) Please have your FACTS ID available to complete the form.


As with all SCRIP purchases you earn both a tuition credit (your family earns half the rebate provided by the retailer on your purchases) and a 1⁄2 hour of volunteer service hour credit for every $100 you spend!
