Town Hall for Members (Virtual)

Developmental Pathways (DP) is hosting a public forum to inform the community about upcoming system changes, answer questions, and address concerns.

This town hall will focus on:

• CFC ( Community First Choice). A new program within the state plan that has functional eligibility element. Specific waiver services will be accessed through CFC. To learn more, please visit, https://www.community.dpcolo.org/community-first-choice

• SIS (Supports Intensity Scale). This is a tool used to determined support levels for various waiver services. The SIS will be decommissioned on July 1, 2025. An interim tool will be available until the new tool is finalized. https://hcpf.colorado.gov/new-assessment-and-person-centered-support-plan

Developmental Pathways (DP) está organizando un foro público para informar a la comunidad sobre los próximos cambios en el sistema, responder preguntas, y abordar inquietudes. Esta reunión comunitaria se centrará en:

• CFC (Community First Choice): Es un nuevo programa dentro del plan estatal que incluye un elemento de elegibilidad funcional. Los servicios específicos de exención serán evaluados a través de CFC. • SIS (Escala de Intensidad de Apoyos; SIS por sus siglas en inglés): Esta es una herramienta utilizada para determinar los niveles de apoyo para varios servicios de exención. El SIS será retirado el 1 de julio de 2025.
