I understand completion of this agreement is required for my child to participate in the 2024 National Conference and is separate and independent of any other forms, waivers, or other documents that may be required by any other group for participation.

I hereby acknowledge that my child and I have arranged for transportation, meals, and proper supervision of my child for the 2024 National Conference. I agree that neither ASTA nor its contractors, vendors or venue are responsible for the safety and oversight of my child.

I hereby acknowledge that I have provided/accounted for all my child’s necessary accommodations (pertinent medical history (physical/mental), allergies, medications, insurance coverage, and any other special accommodations) that may be required for my child to participate in the 2024 National Conference, and have shared any pertinent details with those in charge of my child's supervision.

Finally, I also hereby acknowledge it is the responsibility of my child to properly safeguard their instrument while in transit and during the 2024 National Conference ASTA is not responsible for, and I hereby release ASTA from, any liability for any loss or damage of instruments or the actions or other students/attendees, whether or not they are intentional. I understand it is my responsibility to ensure I have proper insurance coverage for my child’s instrument, or to cover any loss/damage claims. Further, as between ASTA, its contractors or the venue, and myself, I accept responsibility and liability for any claims arising from the actions of my child, whether intentional or not.

I attest that I have fully read and understand all provisions of this agreement and it shall be fully binding on me, and, by extension, my child. If any provision of this agreement is deemed to be invalid or unenforceable, it shall not affect the validity or enforceability of any other provisions. The terms of this agreement shall be governed by and construed in accordance with the laws of the Commonwealth of Virginia.
