The purpose of this form is to determine if your research project needs oversight by the Institutional Review Board (IRB) or the Institutional Animal Care and Use Committee (IACUC). If you have already received approval from either of these committees for your project, you do not need to complete or submit this form. IRB oversees research projects that involve any interaction with humans, along with any research concerning data with identifiable information. If your project involves data, you may be asked to complete a Non-Human Subjects application. IACUC is dedicated to the humane care and use of animals in activities related to research and teaching conducted at UCM. Research is defined, by federal regulation (45 CFR part 46), as a systematic investigation designed to contribute to generalizable knowledge (information usually will be shared beyond those involved in or overseeing the program/process/system).
