Use this form to request a replacement machine from PMACS inventory. Please note the following:

- Computers are only for PMACS supported labs/departments

- Computers are the property of the purchasing University Department

- The machine is expected to be returned to your department.

- No Penn Intellectual Property or HIPAA compliant data can reside on mobile devices and/or must have PSOM security precautions taken

Once the form is submitted, an email will be sent to your BA for authorization for a machine. Your BA should not provide a budget code that uses COVID funds. Additionally, your information will be sent to PMACS support who will reach out to you via the mobile phone number or email address you provide below to proceed.

Returns will be allowed up to 30 days after you have received the computer.
