TEEAC Program Description Form

Providers of educator professional learning and students experiences must complete this form to become a TEEAC Provider. Current TEEAC providers should also complete this form when they develop new programs/workshops.


All fields are required. Once your entry has been reviewed, the program will qualify as hours toward the TEEAC Certificate of Recognition and your institution will be listed in the next update to the TEEAC Providers List.

Provider Information

What is the name of the provider or institution? This name should match the Providers List Update form.

Enter the contact information for the person I should talk to about this program/workshop description, if necessary. It does not need to match the Providers List Update form and can include any of the following:

  • the institution or program website
  • the email address for the presenter, manager, or director
  • a phone number for that person
  • the physical address where the program is held
