SFSP Intent to Operate

Sponsors of the Summer Food Service Program (SFSP) are required to submit, in writing, their intention to operate the program and the sites they plan to serve. Please indicate which sites you plan on operating, as well as any additions or deletions of sites that you know of currently. If you participated in SFSP last year but are not planning on sponsoring SFSP at all this year, please inform our office so that we may send you the required program close-out paperwork.


Please complete this form, one per site. The automated online system should expedite the overall processing time, even though it may take longer to complete the initial request.

Ready-to-eat means the meal pack include food items that require minimal preparation and heating

Bulk means the meal pack include one or more ingredient or partially prepared food items that require more complex assembly and/or heating steps

Answering this question will not necessarily disqualify you from operating a summer food service program site. Answering this question will help to coordinate services and reach more children, while resolving any potential conflicts early.
