Visitor Health Screening – COVID-19 Safety – Prevent the Spread

Mt. San Antonio College recommends all visitors be vigilant about symptom monitoring to identify symptoms early and prevent exposures. The following guidelines will be adhered to by visitors while on campus. This questionnaire is required in order to be physically on campus. 1. Screening is to be complete the morning you are reporting to campus. 2. Avoid sharing of items, or if sharing is unavoidable then hand wash or sanitize after using an item another person has just used. 3. Campus is diligently cleaning high touch areas to minimize your risk. 4. If you are sharing a vehicle masks are required at all times regardless of vaccination status.

Screening Guidelines

The below screening guideline will be used with every visitor while conducting business with Mt SAC on campus or off-site location. Visitors with one or more of these signs or symptoms will NOT be allowed on campus. If you are having symptoms or have been exposed to a positive or been asked to self-isolate or self-quarantine in the past 14 days, reschedule the visit.

Have you tested positive for COVID-19 in the past 14 days?*

Have you or anyone in your home been instructed by a healthcare provider or public health official to self-isolate or self-quarantine in the past 14 days*
If yes, who was instructed to self-isolate or self-quarantine?*
If “someone in my home,” Do they have symptoms or have they tested positive for COVID-19?*
In the last 14 days, have you been in close contact with a person who is a confirmed positive COVID-19 case (within 6 feet for more than 15 minutes)?*
