IRB | Cayuse Account Request Form

In order to access the Cayuse system, you will need to have an account created and authenticated. Provide the information requested below then someone from the Office of Research Compliance will follow up with you.

Enter your Campus Wide Identification Number (CWID).


There should only be nine (9) numbers and do not include any spaces. [example: 889123456]

 

Enter your First Name (or Preferred Name).

 

Enter your Last Name (or Family Name).

 

Enter your CSUF email address.

 

Select your role at CSUF.
