Faculty Review Request Intake

To ensure that faculty reviews are presented in a consistent manner to The University of Texas System Board of Regents, the Office of Academic Affairs has developed this intake form for the submission of the following request:


  • New hires with tenure
  • Endowed academic position appointments
  • Annual post-tenure review
  • Annual new tenure recommendations
  • Faculty workload reports
  • Appointment of institutional administrative officers


Please complete all required fields in this form and submit the required documentation below.

Request Information

Please provide the following information about your request.


For tenure recommendations see the Guidelines for Submission of Tenure Recommendations document.


For new hires with tenure, endowed academic position appointments, and appointment of an institutional administrative officer, submit a request letter and the CV.


For the annual post-tenure review report and the faculty workload report, submit the completed reporting template. The UT System Office of Academic Affairs will send these templates directly to institutions each year prior to the submission deadlines.
