Update Contact Information: Rental Dwelling License
What this form cannot do: This form cannot be used to change ownership. Rental licenses are non-transferable and therefore once a property is sold, the current license needs to be closed and the new owner must submit a new application. What this form can do: This form can be used to either update the applicant info, or change the agent/contact. When needing to change who the agent/contact is, an updated application must be submitted below indicating an update to the license has been made, with the signature of the new contact and notarized. You do have the option to close a license with this form, but you cannot submit for new ownership.
Please provide details on who you are.
What address are the updates being requested for? Where applicable, please include address #, apt/unit #, street name, and directional.
Please provide the email address that can best be used to contact in case of questions or comments.
When needing to change who the agent/contact is, an updated application must be submitted below indicating an update to the license has been made, with the signature of the new contact and notarized.
Any changes to the spelling of names, addresses, and other information concerning the persons on this application must be provided to the Department of Regulatory Services within ten days of the original submitted application. Please fill in both the incorrect and correct information fields ONLY for the information you would like to update. Information you do not need to update (that would be the same in both the incorrect and correct fields) can be left blank. Note: If you are changing the person who is the agent/contact, you must submit an application in the above portal.
Cannot be PO Box or commercial mailing service. Please include address #, apt/unit # (if applicable), city, state, zip, and county.
If you wish to close this application, please tell us why. An example for closing might be that the property is vacant and being sold. Please note that a rental license is required if the owner does not occupy the property. For example, if a relative or friend is living at the property rent free, a rental license is still required.
